Patient Navigators help guide others through the healthcare system, minimizing obstacles that prevent the patient receiving the care and treatment they require. Our team is here to ensure patient satisfaction and quality of care by taking responsibility for:

  • Organizing schedules and managing patient appointments 
  • Facilitating communication between the patient, family members, and healthcare providers 
  • Managing health care records
  • Informing the patient of financial considerations and any available financial resources 
  • Accommodating interpretive language services when needed
  • Managing follow-up visits to ensure continuity of care and access to community resources
